Registration Information for Students New to Dacula High School Students transferring from another school to Dacula High School must be registered through the School Counseling office prior to their attendance. Specific documents are required at the time of enrollment:
Proof of Residency - acceptable documents include two current utility bills (gas, electric, water) OR a rental or housing contract.
Unofficial School Records - include student transcripts, discipline records, and medical records
Birth Certificate - a copy of the student's birth certificate is required for students transferring from out of state schools
Immunizations - immunizations must be on Georgia forms along with a Hearing, Vision and Dental form. All forms can be provided by the Health Dept.
ENROLLMENT MAY BE COMPLETED ONLY BY THE STUDENT'S BIOLOGICAL PARENT(S) OR A LEGAL GUARDIAN. VERIFICATION OF GUARDIANSHIP MUST BE PRESENTED AT THE TIME OF ENROLLMENT.
